Automation: Send Patient Form After Booking Their First Appointment

Instructions on how you can set a campaign to include patient form for new patients

Follow the instructions below to ensure that your patients will receive their patient form immediately booking their first appointments.

  1. Under Form Templates create your Patient Form

FORM QUESTIONS:

Patient Details - Select the information you wish to get from your patient relevant to their details. For more information check the intro article HERE.

Other Questions - Select the Treatment Notes Template you created in Cliniko that has all of the questions that you wish your patients to answer.

OPTIONAL:

  • Copy data to patient notes

  • Body Chart

For Nookal and Coreplus users you will need to add the questions in your Cliniq Apps account.

APPEARANCE:

Design and brand your patient form.

For email form, in edit email create the following email: 

Subject - ACTION REQUIRED: New Patient Form

Hi {FIRSTNAME},

Welcome to our clinic!

It is with great pleasure that we welcome and thank you for choosing us to help assist you. Your appointment will take place on {NEXT_START} with {NEXT_PROVIDER_FIRSTNAME} {NEXT_PROVIDER_LASTNAME} at {NEXT_LOCATION}. Please fill out the following form before your first visit to our clinic. This form will help us get you started on the right track towards your perfect health.

Regards,
Clinic Name

For SMS Form form, in edit SMS create the following SMS: 

Hi {FIRSTNAME},{NEXT_PROVIDER_FIRSTNAME} is looking forward to meet you on {NEXT_START}. Please fill this form before you come.

Clinic Name

ADDITIONAL SETTINGS:

REQUIRED

  • Fill the form for...

  • Email address to receive notifications when the form is filled

OPTIONAL

  • Save a copy of the submitted form as PDF file

  • Add a signature pad at the end of the form

  • Ask privacy policy consent question

Once done, SAVE your patient form and proceed to the next step.

2. Click on Automation Rules and Click on New Automations

Then:

  1. Name the Automation for your reference.

  2. Select unsubscription category

  3. Select unsubscription automation type. For information about automation categorization and type click HERE.

  4. Select the iteration of the automation

  5. Click Continue to proceed to the next steps

Click the SELECT button and choose Patients with Appointments

Select the clinic location and click the and button

Then choose their appointment

And set it to less than 1 month remain before next appointment then click the and button

Select their number of past appointments and set it to 0-0

And click the then button and choose communicate with patient

Select the patient form that you created and set it to 1 hour later

Click on Apply Changes


Then Save Automation

To start the automation simply click the PLAY ▶️ button.

When you activate this automation, your patients will receive the patient form an hour after booking their first appointment in your practice management software.
