Guide to automating patient intake forms in Cliniq Apps for efficient onboarding after first appointment booking.

Streamlining the onboarding of new patients can significantly enhance your clinic's efficiency and patient experience. This guide will help you set up automation in Cliniq Apps to automatically send patient intake forms right after they book their first appointment. It assumes you've already created your Email and SMS forms. If you haven't, please first follow these dedicated guides:


Creating the Automation

Step 1: Start New Automation

  • Go to Automations.

  • Click New Automation.


Step 2: Name and Categorize

  • Name your automation clearly, e.g., New Patient Form (avoid special characters).

  • Choose an unsubscription category, e.g., "A" for Patient Form (Intake), for easy management.

  • Select Patient Form (Intake) as the automation type.

  • Choose One-Time Automation, as initial visits occur only once.

  • Click Continue.


Step 3: Set Automation Triggers

  • Click SELECT and choose Patients with Appointments.

  • Select either a specific clinic location or All Locations.

  • Click the green and button, then select Their Appointment.

    • Change "More Than" to "Less Than".

    • Set unit from minutes to months, and enter 1 month.

    • Then change the first appointment to Next Appointmen (Ensures patients with appointments within the next 30 days join the automation.)

  • Click and again, select Their Number of Past Appointments

  • Set it to 0-0 (ensuring it's their first appointment).


Step 4: Set Communication to Patient

  • Click then and select Communicate with my patient or my team.

  • Click then again and select Communicate with patient.

  • Choose Send Patient Email/SMS.

  • Set the delay to 1 hour later.

Important Tip: Ensure your booking confirmation message (from your practice management software) informs the patient they will receive a form within a few hours and to check their email inbox, including spam folders, if applicable.


Finalizing and Activating Automation

  • Click Apply Changes.

  • Then, click Save Automation.

  • Activate your automation by clicking the PLAY button.


Once activated, this automation automatically sends new patients their intake forms precisely one hour after booking their first appointment, minimizing your administrative workload and reducing manual follow-ups. Cliniq Apps ensures that new patient onboarding is seamless, timely, and efficient, enabling your practice to focus more on patient care and less on paperwork.
