Instructions on how to send a follow-up patient form

Sending a follow-up patient form for patients who haven't filled the first form sent to them yet is as easy as 1,2,3. All you need to do is to create an automation that will automate the same patient form to those who are slipping through the crack. Depending on your internal protocol, you may send the follow up patient form 2-3 days before their appointment and or a day before the appointment. This guide will show you how you can send on the second day prior to their initial appointment.


Simply follow the instructions below:

  1. Go to AUTOMATION RULES in your Cliniq Apps account.

2. Then click on New Automation

Then:

  1. Name the Automation for your reference.

  2. Select unsubscription category

  3. Select unsubscription automation type. For information about automation categorization and type click HERE.

  4. Select the iteration of the automation

  5. Click Continue to proceed to the next steps

Click the SELECT button and choose Patients with Appointments

Select the clinic location and click the and button

Then choose their appointment. Set it to more than 2 days remain before their next appointment. Then click the and button and choose appointment date as shown on the screenshot below.

Set it to 2 days in the future and 3 days in the future. Then click the and button highlighted on the screenshot below.

Then choose their treatment notes & tags

Select the none of these (NOT) option

In the Treatment Notes and Tags filter, apply any word you used in the treatment notes template of your patient form inside the None of these textbox. For example the #How in the screenshot below:

In Cliniko:

In Cliniq Apps:

If the patient has already submitted the form the keyword #HOW is already present in his treatment note in Cliniko. This patient will no longer join this patient group but only those who don't have this keyword. Or in other words those who haven't filled and submitted the patient form yet.

Next step is to click on the and button then choose their number of past appointments as highlighted on the screenshot below.

Set it to 0 and 0 then click the then button

Select the communicate with patient button and select the type of form that you have created

Set the timing of the message

Click on Apply Changes


Then Save Automation

To start the automation simply click the PLAY ▶️ button.
