Save valuable time and enhance patient experience by sending patient forms directly via email. Patients can conveniently complete forms before their appointments, and responses sync seamlessly to your Cliniko or Nookal account, simplifying your workflow and improving accuracy.

Recommended Workflow

Send patient forms via email first. If a patient hasn't completed the form closer to their appointment date, send a follow-up via SMS two days prior. SMS messages have a significantly higher completion rate—open rates are around 98%, compared to 20% for emails.


Step-by-Step Guide

Step 1: Patient Details

The following fields sync automatically to Cliniko or Nookal. Include these fields if you wish to update patient records:

  • First Name

  • Last Name

  • Title (Mr., Mrs., etc.)

  • Date of Birth

  • Email

  • Street Address

  • City

  • State

  • Country

  • Post Code

  • Mobile Phone

  • Home Phone

  • Medicare Number

  • Occupation

  • Emergency Contact Name and Number

Data entered by patients will directly update their profiles.


Step 2: Add Other Questions

You can add customized questions beyond basic patient details. Examples include:

  • Health history and current concerns

  • Allergies or current medications

  • Cancellation policy acknowledgment

  • Marketing insights (e.g., how patients discovered your practice)

Detailed Guides:

Patient answers will be saved as drafts in treatment or clinical notes for practitioners to review and finalize.

Note: Mark questions mandatory by adding an asterisk (*) at the end.

Optional Settings

  • Copy Data to Patient Notes: Automatically save form responses to patient notes.

  • Body Chart: Upload a custom body chart or use the default provided.


Appearance Customization

Customize your forms to match your practice branding by:

  • Adding your clinic's contact details

  • Including a branded header image

  • Choosing colors and styles to reflect your brand
